Medication Therapy Management
Fairview Mesaba Clinic's Medication Therapy Management (MTM) is a service designed to help you get the most from your medicines. Our specially trained pharmacist works closely with you and your doctors to solve any problems related to your medicines and to help you get the best results from taking them.

During a private meeting, your MTM pharmacist will review your medicines and answer any questions you may have. You'll get a list of all your medicines, with detailed advice on how to get the most out of them. Your MTM pharmacist will be available to help you throughout your treatment.

Your certified MTM pharmacist helps your doctors by discussing your medicines and related concerns with you. Your pharmacist and doctors will work together to help you meet the goals of your treatment plan.

The MTM pharmacist also visits patients who are admitted into our hospital as an in-patient.

Patients using MTM are more likely to feel better, be healthier, and get the benefits they expect from their medicines.

Appointments

 Patients can call 218-362-6937 to schedule an MTM appointment with Sara directly, or they can be referred by their physician. However, they do not need to be a patient of a Fairview clinic or hospital to use this service.
