Outpatient Mental Health Counseling

Fairview Mesaba Clinics is proud to offer outpatient mental health treatment and counseling programs to serve our community.  These programs utilize best practices in mental health treatment and counseling to support the recovery of our patients.

Services include, but are not limited to: marriage therapy, individual, couples, and family therapy, domestic violence counseling, sexual misconduct counseling, grief counseling and support, end of life support, diagnostic assessments, anger management, anxiety disorders, personality disorders, behavioral disorders, panic disorder, obsessive compulsive disorders, cognitive behavioral therapy, chemical dependency recovery issues, codependency, EMDR (Eye Movement Desensitization and Reprocessing), PTSD (post-traumatic stress disorder), and others.

Partial Hospitalization Program

What is the Partial Hospitalization Program (PHP)?
Partial hospitalization is an intensive short-term group based program if you’re experiencing an acute episode of depression and/or anxiety. Often, individuals in the program are receiving therapy for the first time and will be transitioning back to work, school or home. Groups meet five hours a day, five days a week, for about two-to-three weeks. You’ll also have regular follow-up with a medication manager. This program may offer a step-down from hospitalization.

Behavioral Health Home

Today’s daily life challenges make it difficult to get the right kind of care when you need it. Now you can receive primary medical and behavioral health care at one location, known as your “behavioral health home,” from a coordinated care team dedicated to serving you.
Fairview’s primary care behavioral health home team works closely together to help you work through day-to-day challenges and move toward healthy living.

Your coordinated care team
At least once a month you will be seen or contacted by someone on your team:
- Behavioral health clinician
- Primary care provider
- Social worker
- Community health worker

Who's eligible?

The Minnesota Department of Health has set the following criteria. Patients must: receive Medical Assistance, PMAP or both Medicare and Medicaid as well as be diagnosed with serious and persistent mental illness, serious mental illness or serious emotional disturbances.

Integrated Behavioral Health

What is Integrated Behavioral Health?

The care that results from a practice team of Primary Care Physicians and Behavioral Health Clinicians, working together with patients and families to provide patient-centered care to meet your medical and behavioral health needs.

A Behavioral Health Clinician is a licensed mental health professional that is available at your primary care clinic to offer support regarding medical and behavioral health concerns and needs.

Services may include:
- Comprehensive assessment
- Brief psychotherapy interventions
- Information, resources and referrals
- Coordination and continuity of care with your primary care team

Who is eligible?

Any patient who has a primary care provider within the Fairview clinic system is eligible.
