HOW TO REACH US
Adult Day Services is a service of Fairview University Medical Center-Mesabi. It is a daytime program dedicated to caring for adults by providing socialization and some health/medical related services.

WHO DO WE SERVE
- Adults with physical, social, and/or emotional needs whose well being is threatened by the effects of social isolation and lack of attention to their various personal needs
- Adults cared for by families or friends whose participation in Adult Day Services may relieve families of 24-hour responsibility, thus providing respite for caregivers
- Persons who reside in our local communities whose disabilities or level of functioning is such that without intervention, institutional placement would be likely to occur
- Individuals who are chronically ill or disabled who can benefit from supportive services to maintain independence
- Hospital and nursing home residents who are able to return to the community with day supervision and assistance

SERVICES
Adult Day Services are individualized to each person's care plan in which participant and caregiver needs are determined in an interview. Various services or combinations thereof are then scheduled to meet the individual's needs.
Activities/Services include:
- Free in-home visit
- One day free visit at ADS
- Protective environment
- Daytime supervision
- Caregiver support
- Nutritious meals and snacks
- Access to internet
- Social and recreational activities
- Community Outings (Shopping, Lunch & Casino trips)
- Emergency medical arrangements
- Liaison with home and community representatives
- Respite for caregivers
- Nursing services
- Transportation liaison
- Medication assistance/reminders
- Educational services
- Structured exercise program
- Special music
- Games & Crafts

WHO IS ELIGIBLE
A person's eligibility is determined individually during an interview process. In addition, each person must meet the following criteria:
- 18 years or older
- Ambulatory or can use a wheelchair or walker
- Not dangerous to self or others
- Can cooperate with own goal plan
- Has a personal primary physician or willing to have one
