HOW TO REACH US
CrossRoads is a service of Fairview University Medical Center-Mesabi. It is a daytime program dedicated to caring for adults by providing socialization and some health/medical related services.

ABOUT OUR PROGRAM
- CrossRoads is open Monday through Friday from 8:00 a.m. to 2:00 p.m., with extended hours available. Participants may attend one day per week, every day of the week or anywhere in between. Attendance is based on whatever schedule is most appropriate for the individual and their family.
- CrossRoads is dedicated to caring for adults through social and recreational activities with some health components.
- CrossRoads provides caregivers with an opportunity for respite. We can offer families peace of mind, knowing that their loved one is enjoying companionship and care while they handle their daily commitments. By staying active during the day and returning home in the evening, participants also keep a sense of independence and remain close to their families.
- Participation in CrossRoads allows the individual to remain living in the community, in the least restrictive environment and may actually delay placement into a skilled nursing facility.
- Payment for these services may be available through the following: Waiver program based on eligibility (county determines), Veterans Administration, respite grant or on a private pay basis.

WHO IS ELIGIBLE
A person's eligibility is determined individually during an interview process. In addition, each person must meet the following criteria:
- 18+ years or older
- Ambulatory or can use a wheelchair or walker
- Not dangerous to self or others
- Can cooperate with own goal plan
- Has a personal primary physician or willing to have one
