SERVICES AVAILABLE AFTER DISCHARGE

The Social Services staff at Fairview University Medical Center-Mesabi are available Monday through Friday from 7:30 a.m. to 4:00 p.m. Their purpose is to provide social work services to the patient and/or family members.

Services Available
These services may include:
  • An interview with the patient and family to discuss concerns such as adjustment to illness and/or hospitalization
  • Information for patients and families regarding access to the Ad Hoc Bioethical Committee. The Ad Hoc Bioethical Committee at Fairview University Medical Center-Mesabi is a multi-disciplinary group of physicians, nurses, social workers, clergy and others who safeguard patient rights and help people deal with medical ethical issues. The committee services patients, family members, chosen representatives, or medical staff and provides a forum in which to discuss doing the "right thing" when their points of view conflict and the debate becomes intense. The committee's goal is to focus on what the patient really wants, or would want if capable of choosing. The committee offers suggestions but does not make treatment decisions. That right is retained by patients, their families, legal counsel and physicians.
  • Early identification of the needs of the patient, so the best plan for discharge can be made by the patient and family.
  • Consultation with nurses, doctors and other hospital staff in order to coordinate plans
  • Assisting with the discharge planning, such as need for placement or home care services
  • Participating in program development and policy planning, within the hospital and within the community

Services Available After Discharge
For the patient who returns home upon discharge, the following resources area available:
  • Home Care Services: which provides registered nurses, home health aides and homemakers for patients needing skilled nursing, personal care and chore services in the home
  • Public Health Services: f or maternal/child care needs or for frail elderly
  • Equipment: on rental or purchase basis
  • Financial Resources: including the local public assistance agency, Social Security Administration, Veterans Administration
  • Home Delivered Meals: for those unable to prepare meals for themselves
  • Transportation Arrangements
  • Arrangements For A Visit To A Patient: who has recovered from a radical breast surgery, colostomy, ileostomy or ileal bladder surgery and who might be able to offer or exchange ideas
  • Hospice Referral: for the terminally ill
  • Information Regarding Community Resources
