Phase II Cardiac Rehabilitation
For individuals with recent (less than three months ago) heart-related diagnoses.
Continuous ECG, heart rate and blood pressure monitoring during exercise sessions provide the basis for learning the principles of an individualized, safe fitness program of conditioning, strengthening and flexibility. An occupational therapist and exercise physiologist supervise exercise sessions. Lifestyle modification education for coronary artery disease risk reduction is also provided. Physician referral/insurance provider approval required.

Phase III Cardiac Rehabilitation
For people who have attended Phase II Cardiac Rehabilitation Program and desire to maintain heart fitness, the Phase III Maintenance Program is available. Cardiac rehabilitation therapist's work with clients to provide intermittent ECG and blood pressure monitoring, reinforce healthy habits, and provide resources and support for ongoing recovery. Physician referral is required.

Diabetes
The Diabetes Resource Center offers a series of group sessions or individual appointments for people with pre-diabetes, type 1 diabetes, type 2 diabetes or gestational diabetes to help you learn how to mange your diabetes. Topics include individualized meal planning, monitoring glucose levels, medication(s) management, how to develop an exercise plan, coping with diabetes, balancing diabetes management into your lifestyle and more. Peri-Operative Education
